Reporting Process

After the Wellness Fund application is approved by the Community Health Board, the applicant will receive the approved funds along with a cover letter that describes the reporting process.

To ensure the funding will be spent as laid out in the Wellness Fund application, the Community Health Board requires a Final Report within 60 days after the project is completed. The final report includes a budget summary and all receipts must be submitted with the final report.
